Audiometry (hearing test)

An audiometrist will conduct a hearing test to determine your level of hearing loss. They can fit hearing aids and provide counselling for those with hearing loss. This service is provided by Bloom Hearing on a visiting basis.

Dietetics and Nutrition

Our Chronic Disease Nurse assists doctors and their patients with their care planning. This is a service for people with chronic conditions, such as:

  • Diabetes
  • Cancer
  • Asthma
  • High blood pressure
  • High cholesterol
  • Heart disease
  • Stroke
  • Lung conditions
  • Early arthritis.

Care planning is a great opportunity to discuss your health – areas that are going well and areas that can be improved – as well as to undertake basic health screening tests.

This process is about setting health goals you want to achieve and developing strategies together to help you improve your health. It is also a good opportunity to assess how the Allied Health Team can support you. For example, with physiotherapy, exercise physiology, dietetics, occupational therapy, social work and podiatry.

Our dietitian provides individualised and practical dietary advice to help individuals of all ages manage their health condition and achieve optimum quality of life. Dietitians can also provide general dietary advice regarding weight loss (or gain) and to prevent long-term health conditions.

Community Nursing

Our community nursing team provides home-based healthcare by offering assistance with hygiene, wound care, medication management, diabetes management, emotional support and monitoring of health status.

This team of highly-skilled nurses can assist you when you first return home from hospital or support you to stay out of hospital.

Availability

Community nurses are available seven days a week, including public holidays. For more information, please call reception on 03 5366 7878.

Occupational Therapy

Occupational therapists can visit individuals in their homes to assess and promote independence and participation in activities for daily living. Our OTs can recommend and prescribe aids, equipment or home modifications, such as grab rails, ramps, scooters and other devices that may assist you to continue to live safely and independently in your home. Our OTs can also provide individuals with strategies to reduce falls and conserve energy.

Who is eligible?

Must be over 65 years of age or living at home with a disability.

Bookings

Appointments are available Monday to Friday from 8.30am to 4.30pm. Occupational therapists are able to visit you in your home. To make an appointment, call the Allied Health reception on 03 5366 7878.

Physiotherapy

Our team of experienced physiotherapists assist in the prevention, treatment and management of movement disorders arising from various conditions and diseases. Physiotherapists also specialise in a number of areas, such as sport injuries, chronic pain, post-operative rehabilitation and sleep disorders.

A 12-week hydrotherapy program is delivered by a qualified physiotherapist. This program involves a tailored water-based exercise program under the supervision of a physiotherapist.

Podiatry

Podiatrists specialise in the prevention, treatment and care of all lower limb conditions and gait pathologies, including painful nail, skin and joint problems.

Podiatrists play a role in the assessment and treatment of the diabetic foot and provide education and advice regarding the prevention of diabetic foot problems. It is recommended that people with diabetes have an annual foot health check with a podiatrist.

We also assess and provide some of the treatments available for children with foot problems, such as toe walking and in-toeing, and for children with foot pain.

At Ballan Health and Care Services, we facilitate optimal foot health to improve mobility, independence and general health and wellbeing.

Appointments are available Monday to Thursday from 9am to 4pm at 164 Inglis Street, Ballan. Please note, we do not offer a home visiting service for podiatry.

Social Work

Our experienced social worker is available to support you with:

Your sessions with the social worker are confidential.

Women's health support program

Having a baby affects you physically, mentally and emotionally. It’s important to look after all aspects of your health, so you can look after your baby.

Our women’s health support program has been designed to help mothers during the ante and post-natal period, and includes:

·         Checking-in with a community nurse – 1x 60-minute session

·         Allied health support, as needed (physiotherapy, occupational therapy, social work, dietetics).

The aim of this initiative is to give mothers and families holistic support that can continue for as long as required after the post-natal period.

Please note: this program is designed to provide further nursing and/or allied health support to supplement any existing interventions you may be receiving from your obstetrician, midwife or maternal child health nurse and is not designed to be a replacement program.

Sessions can be online or in person and any out-of-pocket cost will be based on your level of income.
